We are excited to have you join our training program at JVS! The funding sources that help make this training available to you at no cost, require us to request certain types of information from you. Please know that your personal information will be securely stored and will not be shared with any third party without your explicit consent.

Proof of Authorization to Work in the United States  

To establish both your identity and employment eligibility, you may choose one of the following options for documentation upload:

Option 1 (Submit One Document)
  • Passport OR
  • Permanent Resident Card OR
  • Employment Authorization Card OR

Option 2 (Submit Two Documents)
  • California State Driver’s License + Social Security Card OR
  • State ID Card + Social Security Card OR
  • Voter ID Card + Social Security Card

All Documents must be VALID

Proof of Selective Service Registration

Men born on or after January 1, 1960, are required to register with Selective Service within 30 days of their 18th birthday and up to, but not including their 26th birthday. This includes men who are:
 

  • Citizens of the U.S. 

  • Non-citizens, including undocumented immigrants, legal permanent residents, seasonal agricultural workers, and refugees who take up residency in the U.S. prior to their 26th birthday 

  • Dual nationals regardless of whether they live in the U.S. and/or WIA 101-A Rev. 6/2012  

  • Veterans discharged before their 26th birthday. 

To obtain a printout of your selective service registration you can go to www.sss.gov


Proof of Residency                   
 
Utility bills, bank statements, or public benefits letter showing your address within the last 6 months,
 
OR
 
A postmarked envelope from within the last six months addressed to the participant at your address.

Proof of Household Income 
 
Documentation from an eligible income-qualified benefit (e.g. CalFresh/SNAP, CalWorks, Unemployment Insurance Records, etc). 

Proof of Job Dislocation/Layoff 


1) Notice of termination from a job that you have had a long-term connection to and/or unemployment benefits 


2) Self-Employed - a copy of business license/tax returns & a self-statement which includes information regarding your self-employment status and that it ended due to general economic conditions.
